Change of Name
If you are a current or former student, one of the following document types must be included with your request: 1.) Marriage license, 2.) Divorce decree, or 3.) a Court order.
If the name change request is received without a copy of any of the above mentioned document types, a member of the MSU transcript staff will notify the student concerning the requirement. At that time, the student will have the option to issue a transcript with the last recorded name (possibly a maiden name) or provide the required documentation so that the transcript may be issued with the name change.

If the transcript requestor is bound by a deadline (For example, a new job, etc.) a transcript containing the last recorded name (possibly a maiden name) may be issued immediately. Please Note: If the requested transcript must be issued under the new name, we will not be able to process the transcript request until HRM has processed the name change and has notified the Registrar of the name change. (The Registrar's office will hold the transcript request until HRM notifies us of the name change. If the name on the request must be changed, the Registrar will ask the requestor to give permission to change the request.)
